Lions Sight and Hearing Foundation of Southern California (LSH) collects and maintains personal and financial confidential information, including names, addresses, social security numbers, medical and financial information, corporate numbers, other information and data in connection with its fundraising, public relations programs and other activities. LSH recognizes that under government rules and regulations such as HIIPAA, Medicare, Medi‐Cal, and other state and federal regulations – it is required to preserve the confidentiality of such information and data from disclosure to other persons, entities or the pubic without permission of the affected persons or entities unless required to do so in a legislative, administrative or judicial action.
LSH has acquired such information and data in the following ways:
- From donors of gifts, bequests, legacies, trusts, sponsorships and contributions at its charitable and other activities
- From applicants for financial assistance in its sight and hearing programs and other charitable programs to be adopted in the future
- From Lions and Lions Club Foundations
- From staff, members of the Board of Directors and volunteers in the operations of LSH
- From family foundations, corporations, banks and other legal entities
- From the general public
- From persons who provide information to LSH via email and social networks
LSH recognizes and instructs its staff officers, Board of Directors, part‐ time employees and independent contractors working on behalf of or in the name of LSH to exercise reasonable effort to preserve the confidentiality of all information and data furnished to them or acquired by them while functioning on behalf of LSH, including use of written documents, correspondence, email or telephonic communication.
